Public Housing Application Packet

To apply for the Fort Collins Public Housing program and/or the Wellington Housing program, you will need to download these forms, print them, fill them out completely and return them to the Fort Collins Housing Authority. The following forms must be submitted to apply:

• Public Housing Application Checklist: [Note: Application packets not completed in full will be returned without placement on the Waitlist.]
• Authorization for the Release of Information/Privacy Act Notice [hud-release.pdf]
• Public Housing Application: Signature of head of household required
• Public Housing Personal Declaration: Signatures of every household member over the age of 18 required.
• Fort Collins Housing Authority Participant Responsibilities: Initials and Signatures of every household member over the age of 18 required.
• Declaration of Section 214 Status: One for each person in the household. Parents or guardians are to sign for household members under the age of 18.

When you return this paperwork please present social security cards for each member of the household as well as a state or federally issued photo identification for adults over 18. Download Entire Application Packet:
